• Your Valid EMAIL ADDRESS is Required by T-Mobile. It is used for account verification if you need to call them for assistance.

• Your Phone's 32-Digit EID - Tip: Dial short code *#06# on your device to find the EID and IMEI

• Your Phone’s 15-Digit IMEI

• Your Phone's Make & Model

• Your 5-Digit ZIP Code (used to generate the 3-digit area code for your phone).

• Tip: Dial *#06# from your phone to display the phone’s IMEI 

• After sending the request for esim activation, KEEP THE PHONE CONNECTED TO WIFI and you will receive a notification from T-Mobile to your phone that directs you to install the esim on your phone.
